Who is Eligible for Personal Care Home Health Care?
Anyone who meets the following criteria is eligible for home health
care through Alternative Health Care
:
 | Wishes to remain in their home rather than a nursing home or assisted
living facility |
| Willing and capable of participating in the care plan or has a caregiver
willing and capable of doing so |
Who is eligible for Medication Management:
| Under a physician's care |
| Needs assistance in setting up weekly medication boxes in their personal
home |
| Lives in an Independent Living Center or Assisted Living facility but
needs someone to give their medications at scheduled times |
Who is eligible for Skilled Services:
| Not eligible for Medicare or Medicaid |
| Under a physician's care |
| Able to benefit from skilled nursing care, physical therapy, occupational
or speech therapy |
